What Happens If an AI Recruiting Tool Makes a Wrong Decision?
How to handle wrong AI recruiting decisions: detection, review, candidate remediation, model correction, and the audit log that protects you legally.
AI recruiting tools will sometimes get a decision wrong. The question is not whether, it is what your team does about it when it happens. A clear five-step incident response separates teams that handle errors well from teams that compound them. It also gives you a defensible position with candidates, hiring managers, and increasingly with regulators.
The five-step response
1. Detect
You need a way to know an error happened. There are three reliable signals: a recruiter override that disagrees with an AI rejection, a candidate complaint or appeal, and a periodic audit that compares AI shortlists against retrospective hiring outcomes. The mature posture is to have all three running. A platform that does not give you override signals or audit logs is harder to manage when something goes wrong.
2. Review
Bring a senior recruiter into the loop within 48 hours. Pull the candidate’s record, the AI score, the explanation behind the score, and the comparable candidates who were ranked higher. Decide whether the AI was wrong, the rubric was wrong, or the input data was wrong. Each leads to a different correction.
3. Remediate the candidate
If the candidate was wrongly excluded, re-engage them. A direct outreach acknowledging the situation, a fast-track review with the hiring manager, and an apology that does not over-explain. The candidate experience after a mistake is often more memorable than the mistake itself.
4. Correct the system
Push the corrected decision back into the model as training signal, update the rubric if the issue was rubric-level, and flag the role brief if the issue was input-level. 5. Document
Every override and every correction goes into the audit log: timestamp, candidate, AI decision, recruiter decision, reason. The cost of keeping this log is low; the cost of not having it when you need it is high.

Who is accountable
The honest answer is the company that deployed the tool, not the vendor. Vendors are liable for product defects (the model crashed, the data was lost), but hiring decisions are the employer’s legal responsibility. AI does not transfer that responsibility; it just makes it more important to have controls. This is also the regulatory direction in the EU and several US states.
